Job Summary: We are seeking a dedicated RN Case Manager – Specialty Services to actively manage patient care and coordinate specialty and surgical referrals. This role collaborates closely with providers and interdisciplinary teams to ensure timely, efficient, and patient-centered care. The ideal candidate will demonstrate strong critical thinking, communication, and care coordination skills while supporting positive patient outcomes in a fast-paced clinical environment.
Responsibilities:
Actively manage assigned patients and coordinate care across specialty services.
Collaborate with medical providers and interdisciplinary teams to establish individualized care goals.
Facilitate and track specialty and surgical referrals, ensuring complete documentation and timely follow-up.
Serve as the primary clinical contact for patient care coordination and problem resolution.
Identify barriers to care and proactively work to improve processes and outcomes.
Perform staff nursing duties including assessment, documentation, patient education, and medication administration.
Maintain clinical competencies, infection control standards, and a safe work environment.
Participate in team meetings, quality improvement initiatives, and special projects as assigned.
Requirements:
Current, unrestricted Registered Nurse license (Alaska or other U.S. state).
Direct hires must apply for an Alaska RN license prior to start date.
Graduate of an accredited school of nursing (Bachelor’s degree preferred).
Basic Life Support (BLS) certification required.
Minimum 1 year of nursing experience preferred.
Ability to lift up to 50 lbs and meet physical demands of the role.
